Becoming a Member of Fresh Start Clubhouse is easy and FREE: Member Services and the Guidance Team will guide you through the Orientation process and welcome you to our community. Eligibility Criteria for Membership: 1. Must have primary diagnosis of a severe and persistent mental illness. 2. Must be a resident of Washtenaw County. 3. Must be 18 years of age or older. 4. Must be able to independently meet self-care and safety needs. As individuals are free to come and go as they like from the Clubhouse, they must possess the necessary safety skills and judgement to move around the community. 5. Cannot currently pose a threat to Clubhouse community or themselves. 6. Must indicate and interest in rehabilitation/recovery goals. 7. Of those that have a history of substance abuse, must be clean and sober for at least 90 days. 8. Has a Case Manager through CSTS (Community Support & Treatment Services) | Steps to becoming a Member 1. Set up an appointment for Orientation 1 -Orientation I occurs every Wednesday at 10 AM 2. Go to Orientation 1 -Receive a tour of Fresh Start -Participate in a presentation that will teach you about Clubhouses and how Fresh Start works -Fill out Prospective Member Form 3. Set up an appointment for Orientation 2 -Schedule this on any day M-F with the Member Services Coordinator 4. Go to Orientation 2 -Read over the Safe Clubhouse Community Policy -Discuss your personal goals for recovery -Find ways that Fresh Start can act as a tool to aid in your recovery 5. Sign the Membership Application 6. You are now an official Member! |
